If you have a Notice ID and PIN, use the form below.

If you received a notice via mail or email, you have been assigned a Notice ID and PIN. If you are having trouble locating your Notice ID and PIN, please contact the Settlement Administrator for your Notice ID and PIN.

If you cannot find your notice received via mail or email, or if you cannot locate your Notice ID and PIN, you may still submit a claim without a Notice ID and PIN after confirming your eligibility as a Class Member. If you have a Notice ID and PIN, do NOT use this form.
